Residents are matched to a suitable home through a person-centred approach that considers their individual needs, preferences, and long-term goals. Embrace works closely with each person and their support network to ensure the chosen home aligns with their lifestyle and level of independence, within a flexible continuum of care.
Embrace Community Services offers HIQA‑regulated, high-quality residential and community support, underpinned by robust clinical governance and internal quality assurance. Each home is overseen by a designated Person in Charge (PIC), who ensures compliance with standards and leads monthly quality reviews. Nursing care is provided by qualified nurses who manage residents’ medical needs, create individual health plans, oversee medications, and guide support staff . This approach ensures residents receive person‑centred care tailored to their needs, whether in fully staffed residences or supported living arrangements.
Embrace actively involves families at every stage—working closely with them during the move-in process and maintaining their engagement throughout. From planning and preparing the home environment to ongoing communication and support, families remain central to care decisions, ensuring continuity and collaboration in the individual’s journey.
Embrace residents enjoy a wide range of community-focused activities – from shopping outings, exercise sessions, and social gatherings, to volunteering and giving back locally. These engagements are part of a holistic, strengths-based approach designed to support independence, personal growth, and meaningful community participation.
Embrace develops personalised care plans through a collaborative, person-centred process. Qualified nursing staff carry out comprehensive assessments to identify each resident’s health needs, goals, and support requirements. These plans are created in consultation with the resident, their family, and the caring team, then implemented by the designated Person in Charge (PIC) and nursing staff. Care plans undergo regular review – nursing assessments are updated as health needs change, and PIC-led monthly quality assurance reports are evaluated by Embrace’s Clinical Governance Board to ensure care remains relevant, effective, and high-quality.
Referrals are made through the HSE, usually by a Case Manager or Disability Manager following an assessment of needs. Families can contact their local HSE office to begin the process.
